A progress payment lien waiver covers one draw or billing period. It does not close the whole job. Later draws stay open unless the form itself says otherwise.
Send it with a progress pay application, for the amount and dates on that draw. Keep retainage and unpaid extras off this waiver unless your form already includes them.

What does a progress payment lien waiver cover?
